Dear Crestview Parents/Guardians,
Avita Orthopedics & Sports Medicine will once again be offering FREE sports physicals for student-athletes at contracted schools, including Crestview Local Schools.
These physicals are available for students entering grades 7–12 for the 2026–2027 school year and will satisfy the OHSAA pre-participation physical requirement for athletics.

Please note:
Appointments are required – no walk-ins will be accepted
Sports physicals are offered on a first come, first served basis
Students should arrive 10 minutes prior to their scheduled appointment time

Step 1 – Sign Up for an Appointment

Registration Deadline: June 5, 2026
Step 2 – Complete Required Forms
Students must bring a completed 2026–2027 OHSAA Physical Evaluation Form with all required signatures to their appointment.
The form can be downloaded on the last section of your student profile in FinalForms.
Physicals will be performed by a team of Avita medical providers, athletic trainers, nurses, therapists, and volunteers.
