We've started preparing for the 2023-2024 school year and we need some information from our returning families. Please log into FinalForms (https://crestviewashland-oh.finalforms.com) to review and update your child's information for the fall. This information must be updated annually. Be sure to select the 2023-2024 school year.
Now is a great time to update everything, but at a minimum, we need the following by May 15th:
- Transportation information - this information will allow us to effectively build and communicate our bus routes before school begins
- 7-12th Grade Sports & Summer Extracurriculars - Ensure your child's coaches and directors have current medical information for volunteer and summer rehearsals.
