Parents:  Please review the following items to help you understand some of the key concepts, expectations and regulations in regards to our education based interscholastic athletics program at Crestview High School.

OHSAA Preseason Power Point:  Parents please review the slide show put together by the OHSAA in regards to important requirements set forth by the OHSAA for your student athletes participation in interscholastic athletics.

Athletic Paperwork:  All paperwork is to be filled out through Final Forms which can be found on our school website.  No athlete will be permitted to officially play in an athletic event until all paperwork, and , fee’s have been received.

OHSAA Mandated Athletic Physicals - NO athlete will be permitted to practice or participate in an athletic practice or event without an UPDATE Sports Physical.  Physicals are good for 13 months.  The Athletic Department must have a copy of the physical on file.

Athletic fee’s:  Athletic fee’s can be paid using K12 Payment Center which is found under the Parents link and Food Service page on our school website.  Once logged into K12 select the School Fees tab on the left hand side, scroll down to the drop down menu and select Available Fees.  Parents may also purchase Athletic Passes from the K12 payment center.  If this option does not work for you please make checks payable to Crestview Athletics.

Sportsmanship:  Please follow the Chain of Command if there is any issues that may arise during the course of the year.  Communication is key so please don’t hesitate to reach out using the proper channels.  The Chain of Command procedure can be found in Final Forms.
