Children who will be eligible to attend kindergarten for the 2021-2022 school year must be 5 years of age by August 1, 2021. Kindergarten registration will take place from March 15-26, 2021. Parents need to call the board of education office at 419-895-1700, ext. 17002 or 17000 to inquire about the registration process.
Information required for registration is: Birth Certificate, current Immunization Record, Custody Papers (if applicable), and Two Proofs of Residency in the Crestview School District. For a complete list of acceptable proofs of residency, visit our website at www.crestviewschools.net, click on “Registration” link. Once the required documents have been presented, you and your child(ren) will be set up in Final Forms, our online enrollment system. You may complete these forms from home or in our office if you don’t have access to a computer at home.
Current preschool parents need to notify us if you’re planning to send you child to Kindergarten in 2021-2022 and complete the online forms in Final Forms. You will also need to call to set up a kindergarten screening appointment.
Screening appointments will be made at the time of registration. Please mark these dates on your calendar if you have a child who will be entering kindergarten in the fall. We need to get all students registered in March in order to plan for the 2021-2022 school year.
